Refine from Within

Fitness Class Waiver

I am aware that my fitness class will involve up to 60 minutes of physical activity, which will place stress on my muscular and cardiovascular systems. I acknowledge that some classes will be taught via a remote location, and I am solely responsible for the safety of my physical space in this remotely instructed class.  Should the class exceed my physical ability, I understand that I am cautioned not to overwork my body and to do only the movements I am physically capable of executing. I understand that I must be in good health to participate in fitness classes with Refine from Within LLC. I understand that before participation in any fitness class with Refine from Within LLC, I must receive medical clearance from my physician or midwife, clearing me for participation in physical activity. 

I agree to notify the Refine from Within LLC instructor regarding any physical condition that I may have which could be affected by my participation in fitness with Refine from Within LLC. I understand that I am solely responsible for supervising and guarding my health and safety.

I hereby release and waive for myself, and on behalf of my child(ren), my heirs, my executors and administrators, any and all rights to claims from damages arising from any illness, injury, occurrence or aggravation to myself or my child(ren) as a result of participation in or connection with Refine from Within LLC. This release and waiver covers Refine from Within LLC, and facilities at which Refine from Within LLC classes are held.  I agree to hold Refine from Within LLC free from any and all liability whatsoever arising out of this event including but not limited to: physical injuries, muscle strains, tears, pulls, broken bones, miscarriage, death, COVID 19, viruses, bacterial infections and any and all illnesses.

I further agree to all conditions of registration including but not limited to a No Refund policy. I have read and understand the Refine from Within LLC program policies. I understand that the program policies may change at any time and are available for my viewing at www.refinefromwithin.com. I understand that I may be contacted from time to time regarding Refine from Within LLC classes, events or programs, and I have the right to request to be removed from a contact list. 

I hereby grant to Refine from Within LLC and it’s legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs of me, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium when my prior consent is requested; to alter the same without restriction; and to copyright the same. I hereby release Refine from Within LLC and its legal representatives and assigns from all claims and liability relating to said photographs.
